Deputies say Chad Johnson, 49, was last heard from by family members by phone on the evening of June 23.

Johnson is described as 5 feet 11 inches tall, approximately 150 pounds, and has thinning brown hair and brown eyes. He often wears a bandana on his head.

He may be in the area near I-75 and Collier Boulevard. Family members are concerned for his wellbeing.
