Gerardo Williams Diaz, 63, who police described as a black male standing 5-foot-9, weighing 210 pounds with brown eyes and a bald head, was last seen in his room at an assisted living facility located at 637 S.E. 12th Court, Tuesday, Sept. 3, at 4:15 a.m. During a check at approximately 5:30 a.m., officials said, it was discovered Diaz was not in his room or in the residence.

According to the CCPD, Diaz suffers from advanced Alzheimer's and is believed to be on foot traveling in an unknown direction. He was last seen wearing a green shirt and tan shorts, and he is right-handed.

The department initially issued an alert about Diaz Tuesday morning.

During a check at approximately 5:30 a.m., it was discovered Diaz was not in his room or in the residence. He is believed to be on foot traveling in an unknown direction.
