Details are sketchy but hikers have mentioned traffic jams on Half Dome all this season (and for the last couple). Slow hikers or hikers who clip into the cables cause big traffic jams. Many people really want to get down, quickly (some didn't bring enough water, or whatever). More falls happen on the way down at any rate.
Some people decide to step just outside the cables in order to go faster (and just use one cable as a kind of banister). If she in fact was inside both cables, she's one very unlucky woman who lost her footing (almost certainly had to be going faster than safe) and slid under the cables.
Fell 500 feet. Same thing happened last year.
