Boston Globe:

A Pittsfield man who was found dead along with the body of his 11-year-old daughter in his apartment in August died of a heroin overdose, but what killed the girl remains a mystery.

Reports from the medical examiner indicate that Anthony J. Nazareth, 47, overdosed on heroin, but the cause of death for Hannah Nazareth is listed as undetermined, Berkshire District Attorney David F. Capeless’s office said.

The statement said that Hannah probably died “as much as a month before her father” and that Anthony Nazareth “provided false and misleading information” about her whereabouts to different agencies and individuals in July.

Prosecutors said none of the unspecified “agencies or individuals followed up to determine where Hannah was and if she was safe and healthy.”

So incredibly sad. Sounds like the best friend and her mom were great supports for this little girl. Addiction is so awful.
