Police said they believe Eddie B. Horace, 48, walked away from the Adult Foster Care Center on the 14500 block of Rosemary Street in Detroit on Oct. 9.
Horace's care providers said is in poor mental condition and suffers from schizophrenia.
Horace is a black man, 5 feet 3 inches tall, weighing 150 pounds, with black curly hair, brown eyes and a medium complexion.
He was last seen wearing a gray sweater jacket with a hood, white shirt, blue jeans, tennis shoes and a dark-colored baseball cap.
