CARBONDALE, Ill. (KFVS) - Illinois State Police activated an Endangered Missing Person Advisory for a man last seen in Carbondale on Tuesday, August 13.

Police are asking for the public's help in finding a missing man out of Carbondale.

Slomkowski is described as 5'5" tall, weighs 125 lbs. with blue eyes and gray hair. He was last seen wearing a blue shirt, black pants with black and white shoes.

Police say Slomkowski has a medical condition which places him in danger.

CARBONDALE –The City of Carbondale Police Department continues to seek the public’s assistance in the search for Richard E. Slomkowski., age 67 of Carbondale who was last seen on August 13, 2024, at 7:00 p.m. in the area of West Main Street and New Era Road in Carbondale. Richard, pictured below, is 5’05” tall, weighing 125 pounds, with blue eyes and gray hair. Richard was last seen wearing a navy blue shirt, black pants, and black and white shoes Richard has a condition that places him at risk. Anyone with information is asked to call the police department at the number listed below. The investigation is active and ongoing.

Circumstances of Disappearance On August 14th, 2024, Slomkowski walked away from MHC (Memorial Hospital) Slomkowski was located on Carbondale Police video walking from the area, interacting with citizens. He was last seen near the 2100 block of W Ramada Ln in Carbondale, IL. Air resources, drones, and all-terrain vehicles have been used in the search. A search and rescue crew and canine group have canvassed the area with no luck. A ping of his cell phone indicated the last known location of the phone was in the 1600 New Era Rd, Carbondale, IL.

CARBONDALE — The City of Carbondale Police Department has continued to search for missing person Richard E. Slomkowski, 67, of Carbondale, who was last seen on Aug. 13, 2024.

"responded to a rural area between North Oakland Street and New Era Road for a report of a deceased person. The investigation revealed the deceased person was Richard Slomkowski.

No foul play is suspected at this time and his death is believed to be accidental. The investigation into the incident continues to be active and ongoing."
