How tragic. It’s called Deadliest Catch for a reason. So many ways to get injured and so many ways to die on a crab boat. May the RIP. Their poor families. Something like this affects the whole fishing community. It reminds them that this could just as easily been them. JMO

Coast Guard convenes investigation board for Scandies Rose sinking | National Fisherman

The Coast Guard announced Friday it is convening a Marine Board of Investigation – its highest level of official inquiry — into the Dec. 31 sinking of the fishing vessel Scandies Rose with the loss of five of its seven crew members.

According to the Coast Guard, investigation board members looking into the Scandies Rose loss must decide:
  • The cause of the casualty, including the cause of any death.
  • Whether an act of misconduct, incompetence, negligence, unskillfulness, or willful violation of law committed by any individual licensed, certificated, or documented has contributed to the cause of the casualty, or to a death involved in the casualty, so that appropriate remedial action may be taken.
  • Whether an act of misconduct, incompetence, negligence, unskillfulness, or willful violation of law committed by any person, including an officer, employee, or member of the Coast Guard, contributed to the cause of the casualty, or to a death involved in the casualty.
  • Whether there is evidence that an act subjecting the offender to a civil penalty under that laws of the United States has been committed, so that appropriate action may be undertaken to collect a penalty.
  • Whether there is evidence that a criminal act under the laws of the United States has been committed, so that the matter may be referred to appropriate authorities for prosecution.
  • Whether there is need for new laws or regulations, or amendment or repeal of existing laws or regulations, to prevent the recurrence of the casualty.

Coast Guard seeks information on deadly New Year's Eve sinking of Scandies Rose

Coast Guard investigators would appreciate anyone with information about the vessel or conditions around the time the ship was lost to come forward, according to Petty Officer Janessa Warschkow.

“Whether that is former sailing experience on board the Scandies Rose, experience with the crew of the Scandies Rose, if you know the weather between Chiniak and Kodiak on December 31 of 2019,” she said. “Any information is helpful for the ongoing investigation.”

The Coast Guard has released very little information about its findings so far. And according to Warschkow, the investigation could take months or even more than a year to conclude. Eventually, she said, the agency will release its findings.

Settlement in deadly sinking of Scandies Rose crab boat calls for more than $9 million payout

The owners of the Scandies Rose have reached a settlement of more than $9 million with two surviving crew and the families of four men who died when the Washington-managed crab boat went down Dec. 31 off Alaska.

An inquiry into the sinking is underway.

A Coast Guard Marine Board of Investigation was launched earlier this year. After delays because of the pandemic, public hearings are expected to be held in the Seattle area for a two-week period beginning Feb. 22, with the participation of the National Transportation Safety Board, according to Cmdr. Greg Callaghan, the Coast Guard chair of the board.

Callaghan, the chair of the Coast Guard board, said the investigation is exploring a range of issues that could have contributed to the sinking, including the icing conditions’ impact on vessel stability and mechanical or other problems the vessel may have experienced.

The board will examine what pressures the crew were under due to fishing regulations. The board also will look at crew training, and how the lifesaving equipment performed.

NTSB announces the probable cause of the sunken Scandies Rose

Following the year-and-a-half investigation into the fatal sinking of the Scandies Rose, the National Transportation Safety Board unanimously approved the investigative team’s findings on the probable cause for its 2019 sinking on Tuesday morning. [snip] The NTSB said the vessel’s inaccurate stability instructions resulted in a low margin of stability to resist capsizing. That combined with heavy lopsided ice accumulation due to wind and sea conditions, which were more extreme than forecasted during the voyage, caused the vessel to sink near Sutwik Island. [snip] The board highlighted 12 findings the investigative team believed to have played a role in causing the ship to capsize. The findings had included factors such as the ice buildup and inaccurate stability instructions. Those findings also excluded several factors from the incident, such as the captain’s decisions before departing, operational pressures, fatigue as well as drug and alcohol use. [snip] “The Scandies Rose didn’t capsize and sink because a crew member or the captain did not do their jobs, or because it had been poorly maintained,” Sumwalt said in his closing statement of the board meeting. “It sank because its captain only had partial access to the information that he needed to make the right decisions, and the information that he did have was inaccurate.”

The NTSB issued seven recommendations to the Coast Guard, NOAA and fishing vessel operators. Its report calls for more accurate stability instructions that realistically consider the effect of icing on crab pots; a review of the accuracy of stability booklet calculations for the uninspected crab boat fleet; better stability training for commercial fishing crewmembers; and a requirement for personal locator beacons (PLBs) for all crewmembers.
