Suisun City police are asking for the public's help in locating Dianna Freitas, who was reported missing by her family.

Police say she has not been seen for a few weeks and family members are concerned due to an unspecified medical condition.

Freitas reportedly has a history of disappearing for weeks at a time and staying in homeless shelters.

She is known to frequent Fairfield, Suisun and Vallejo.

She was last seen near Armijo High School in Fairfield.

Freitas is described as white, 67 years old, 5 foot 5 inches tall, 175 pounds, with white hair and hazel eyes. She wears eyeglasses.

She was last seen wearing black spandex pants, a shirt and a wooden bead rosary with a cross.
