This person is very close. The only thing I see is place of birth is Kentucky and place of issue for the SS# is California, but that long ago means nothing really.

When I trace the Thelma who is married to Morris in the 1940 census, I find that her maiden name was Cooper based on the children's birth records but I am having trouble verifying that this Thelma's last name is Cooper.
