(Reuters) - Iran's health infrastructure is strong and ready to cope with a possible escalation in coronavirus cases, President Hassan Rouhani said Saturday on state TV.

Iran is among the few countries worldwide severely affected by the pandemic. On Friday, the health ministry reported a death toll of 2,378 and total confirmed infections at 32,332.

Now, however, Hong Kong is providing a very different object lesson -- what happens when you let your guard down too soon. The number of confirmed cases has almost doubled in the past week, with many imported from overseas, as Hong Kong residents who had left -- either to work or study abroad, or to seek safety when the city seemed destined for a major outbreak earlier this year -- return, bringing the virus back with them. On Monday, Hong Kong Chief Executive Carrie Lam announced that all non-residents would be barred from the territory as of Wednesday, the latest addition to a raft of new measures.

This is a pattern playing out across parts of Asia -- mainland China, Singapore, Taiwan -- that were among the first to tackle the outbreak. All are now introducing new restrictions as a sudden wave of renewed cases begins to crest.

BELLINGHAM – An emergency room physician who publicly decried what he called a lack of protective measures against the novel coronavirus at his workplace, PeaceHealth St. Joseph Medical Center, has been fired.

Lin said supervisors threatened his employment more than a week ago
after he spoke to reporters and made social media posts accusing PeaceHealth of a lack of urgency to protect health care workers from the virus.

Lin said he was told to take down his social media posts about the hospital but refused.

He continued to post daily updates on Facebook after shifts at the emergency room, although many of his posts had shifted away from hospital practices to efforts to help secure more protective equipment for hospital workers.

Specifically, Lin had written that PeaceHealth St. Joseph refused to screen all patients outside the hospital, rather than in an often-crowded emergency room waiting area where the virus could easily spread. Two emergency department workers, who both asked not to be named for fear of reprisals, told The Times they shared Lin’s concerns about the possible spread of infection because of that practice.

Lin and other doctors have also persistently complained about the availability of testing approved by PeaceHealth, even as testing capacity ramps up in Washington state.

Hospital administrators this week announced a series of protective measures, such as temperature screening of staff entering the building, plans to enhance separation of staff from infected patients, and the availability of tents to conduct outside screening if deemed necessary.

Lin and other hospital staff noted that most or all of these measures came after Lin’s treatises prompted a community outcry. Meanwhile, Lin maintains the measures fail to meet standards set by other regional hospitals and even smaller health care facilities.

PeaceHealth St. Joseph is the only emergency facility for some 250,000 people in the state’s northwest corner.

Several” hospital staff have tested positive for the virus, the hospital’s chief executive, Charles Prosper, announced this week, insisting that the infections were unrelated to their work at the hospital.

Whatcom County to date has recorded 92 positive COVID-19 tests, a large number of them at a single facility, Shuksan Healthcare Center in Bellingham’s York neighborhood. County health officials said Thursday that the virus had spread to “several” other local senior facilities. Four county residents have died.

Lin said he has been touched by a groundswell of support from residents and health care workers in Bellingham.

“I’ll be OK,” said Lin, a longtime physician whose ER work included a stint at a trauma center near the World Trade Center in New York City during the 9/11 terror attacks. “It’s a blow to my ego more than anything.”

American Association for Respiratory Care, The Society of Critical Care Medicine, American Society for Respiratory Care, American Society of Anesthesiologists, American Association of Critical-Care Nurses and many other groups have come out against use of the valve which allows multiple people to use one ventilator. I did a screen shot of some of the reasons they listed.

Microbiology experts in Spain have said that rapid coronavirus tests that the country bought from China are not consistently detecting positive cases.

The error was discovered as Spain is in the grip of one of the worst coronavirus outbreaks in the world, second only to Italy in the number of reported deaths.

Studies on the tests done in Spain found that they had only 30% sensitivity, meaning they correctly identify people with the virus only 30% of the time, sources told the Spanish newspaper El País.

Those sources told the newspaper that the tests should have a sensitivity of more than 80%. The US Centers for Disease Control and Prevention says rapid tests for influenza are required to have 80% sensitivity.

Rapid tests can get a result in minutes but are generally less accurate than other tests. Numerous companies around the world are producing them.

Sequenced cases from elsewhere in the US and Canada are sometimes related to this transmission chain, but most (77%) derive from separate more numerous and smaller introductions. Next up, 201 genomes from the United Kingdom. These show a different pattern in which there are multiple clusters of related viruses, suggesting repeated introductions into the country followed by local spread. auspice 8/14

A look inside the Javits Center’s new coronavirus hospital

The makeshift hospital, dubbed the “Javits Center Medical Station” by FEMA, is set to open on Monday.

Each room is outfitted with a bed, lamp, night table and humidifier to ease the breathing of patients suffering from the deadly respiratory disease.

There’s hand sanitizer — made by state prison inmates — on the night table, and each room is also equipped with a red backpack that holds an N95 respirator and more hand sanitizer for use once the patients are discharged.

Iran's coronavirus cases surpass 35,000; death toll over 2,500

Iran has reported 139 more coronavirus-related deaths, bringing the total to 2,517.

At least 3,076 more cases were also recorded, bringing the total number of infected people to 35,409.

The US believes the real numbers of cases and deaths in the country are higher than those being reported.
