Inside the "heart" are what look like letters through the center, a pattern that could be a flower below the seven of the ruler and a rounded edge outside the heart, the corner pointing towards the livid pinky.
As a body becomes rigid, anything pressed against it leaves an impression. If I'm not just seing things, or if what I'm seeing has nothing to do with any post-picture tampering or whatever else, it could speak to a personal relationship. Placing something in the hand of a deceased loved one. Then the understanding of that gesture as something done with love by the one who found the body, removing the item to keep it from any other understanding eyes.

that definitely seems plausible. that does not look like a heart to me personally.
