Authorities say a statewide Silver Alert has been issued for a missing 74-year-old man from Portage who was last seen Monday night (April 1st) at 11 pm and is believed to be in extreme dangerous and may require medical assistance.

Authorities have issued the following statewide Silver Alert:
he Portage Police Department is investigating the disappearance of H. John Blumenthal, a 74 year old white male, 5 feet 9 inches tall, 170 pounds, gray hair with brown eyes, last seen wearing a green shirt with a stripe across the middle, jeans, black shoes, has an eagle Navy tattoo on his right arm, a scar on his back, and an appendix scar. H. John was last seen driving a silver 2010 Chrysler Sebring convertible with a black top and with Indiana plate 512LWZ.
H John is missing from Portage, Indiana which is 154 miles northwest of Indianapolis and was last seen on Monday, April 1, 2019 at 11:00 PM. He is believed to be in extreme danger and may require medical assistance.
