Missing From: Mastic Beach, New York

Missing Since: May 21, 1998

Age: 20 -- Height: 5'8" -- Weight: 183 lbs -- Hair Color: Black -- Eye Color: Brown

Jack's nose and tongue are pierced. He has a scar on the left side of his forehead.


Jack was last seen at 10:30 pm in Mastic Beach, New York on May 21, 1998. He was getting into a vehicle at the time. He has never been seen again.
