Back in the day the formal way of addressing a married woman was to write Mr and then the woman's name. So we could be looking for a contact that is indeed a woman.
I thought it was Mrs. (Husband's name) Last name. No??

Arthur J Roznowski of Bay City, Bay County, Michigan was born on January 12, 1927, and died at age 73 years old on June 13, 2000.

Warren F Roznowski of Alpena, Alpena County, Michigan was born on June 10, 1928, and died at age 82 years old on November 4, 2010.

Warren was married to
Arlean Isabel Roznowski, 86, of Alpena. Passed away Monday, February 24, 2020. He passed in 2010. They had 2 sons, Rick and Randy.

Decedent passed in '72?? So, we can cross of Warren. But he might be one of the sons referenced in the note.

A few random observations--"Mr. Phyllis"---Mr implies male, but Phyllis is traditionally a female name. Jefferson St. is nowhere near Keller. And the building pictured in google maps is 8981--8986 would be across the street and is an empty lot.
West Jefferson Ave is very close to 8986 Keller Street, Detroit, MI 48209, it is the main city road one would likely take to get to that address if heading westbound. You would take W Jefferson Ave until it turned into Dearborn St and then turn onto Keller St from there.

On www.historicaerials.com/ the block of 8981-8986 appears to be fully intact in 1973 and 1983, but by 1999 many houses have been torn down. I can't tell if 8986 Keller is one of those torn down by 1999.

You need to zoom in to Keller St in the satellite view Google maps, find the property number of 8986, with house thereunder, it's evidently (do a comparison of the roof features, the grass non-concrete driveway with the entry via the gap in the front chainlink fence, the heavy foilage on the property east boundry fence, the abrupt cessation of the cement footpath at the property northeast corner [grass has actually overgrown the footpath]) - it's the same house seen in google maps streetview that is noted (as at the date of this post) in googlemaps streetview as 8987. Possibly google maps incorrectly recording the house number 180 degrees out of sync with the actual houses on the ground, I've seen this occur so often it's not unusual. I'm on a mobile, are there any other years available to check the streetview for Keller St, maybe someone can zoom in on the house refuse bin & perhaps the street number is visible on it, to better prove this for you?
Incidently, someone seems to be living in that house as at the month/year of the current streetview, note the glow of the electric light seen to be on behind the curtains of the house front window.

I figure the deceased was possibly a hobo type, did some work for the house residents, made a note for hopeful future work with some points to help his memory (e.g. nearest known to him familar road ' W Jefferson', e.g. most memorable thing about that property 'two sons' that perhaps followed him everywhere as young curious scallywags would when you're working on a property).

No responsible agency cited in the NAMUS contact investigating agencies section, doesn't suggest local agencies are following this matter up.

P.S. I went down the rabbit hole of a handful of various other reasonable spellings of both surname & some of the 1st name, possible relatives of the name on the paper (e.g. there's a Balvina Rosowski & husband buried in the Catholic cemetary off to the north...) probably the best hope is to let whatever agency has responsibility for this to make enquiries of the house & property records, to hopefully eventually figure out the actual correct spelling of the name of the lady that resided there.
