I have been trying to think what is the item she WASN'T wearing that was mentioned in the note. I know watch has been thrown around, but I don't know. Is there something else that would be very obvious to those who knew her that she wasn't wearing? It couldn't be a specific piece of jewellery unless they knew her well and she usually wore it. Family might be able to confirm this.
I doubt its a piece of clothing, but did she wear a wig or hair piece?
A wig or a hairpiece is a good idea. Maybe, a hearing aid?
Also, her pacemaker was synchronized with Apple, they say. Could it be Apple watch? Did they leave it behind?.

If so, it might be a better thought over plan than I thought.
"Proof of life". The relatives said that Nancy Guthrie was on the medication that she needed to take daily. With the pacemaker, I'd hazard, it could be a blood thinner. Might be something else, but blood in the house also might point at Nancy being on an anticoagulant.
So, depending on coagulation status and sheer luck, missing one day may be fatal or a person could stay off it for a week.
If they left an Apple watch at home and it was synchronized with the pacemaker, maybe this is all that they said.
They didn't want anyone to get "proof of life" from the devices when they were abducting Nancy. In fact, the whole ransom note is based on no one being able to check Nancy's status.
So. My dad had afib and a pacemaker. It was an old model, put in years ago, not synchronizable with anything but a very good one. All I know about the devices is just from this, and similar, experiences. I am not a cardiologist nor a nurse that sees these things daily.
But: these people seem to have understood the risk they were taking.
What does it tell us:
They have some experience with pacemakers. They are not some unhoused vagabonds, they probably understand the principle of synchronizing with the Apple watch, or can figure it out fast. They saw it somewhere. Either they are in a medical field, or a relative has a similar device. Or a YouTube video.
They probably knew that Nancy had a pacemaker and was on meds? Perhaps? Likely?
Now the sheriff said something about the camera across the street being out of order? So, they were prepared.
Just my wild guess - they probably demanded a very high ransom but... not insurmountable for the family.
They have a viable cryptocurrency account.
Now about an anticoagulant. Nancy is an old woman, probably, on Medicaire but with her daughter being an earner, can afford something better than Coumadine. Such as Xarelto or Eliquis.
(What she takes makes a difference. I am not explaining why here.)

They are in a horrible rush, because they abducted an old woman who is physically fragile. Hence, ransom notes to TMZ and the police.
So I think that they somehow know Nancy, maybe not close but are aware of her issues and understand these things.
They are probably not kids but young enough to deal with cameras, cryptocurrency. Sturdy as to carry a person of her height/weight out takes some strength.
Either their relative knows Nancy but not close or they have an older relative with similar issues.
What comes to my mind... you know, the pharmacies offer delivery of meds to the house? Uber or Lyft?
Could Nancy, living alone and at 84, use a local pharmacy and have meds delivered to her house?
I don't think the case is political. I don't think it is sexual. I think it is for ransom, that the person perfectly well knows who she is and the family's situation but is not someone on their radar.
I'd check the medical aspects. Who was aware and understood the nature of her problems.
I don't know if it is late or not. But I think they can be figured out.
