The N.C. Center for Missing Persons has issued a Silver Alert for a 73-year-old man.
Leonard Mathew Fooks was last seen at 1633 Ashley School Circle and suffers from a cognitive impairment, according to the alert.
He may be traveling via a Winston-Salem Transit Authority bus and is possibly headed to the Winston- Salem Downtown Health Plaza.
Fooks is 5 feet, 9 inches tall, weighs about 180 pounds and is bald. He has brown eyes and was last seen wearing a black jacket and blue jeans.
Anyone with information about Fooks is asked to call W.E. Kelsey at the Winston-Salem Police Department at 336-773-7700.
