Following Martin’s disappearance, DHHS launched an investigation and found Integrated Behavioral Health Services was in violation of a number of policies on April 5, when Martin walked was reported missing.
10/11 News obtained the results of the investigation and corrective actions in a public records request. The documents are partially redacted and don’t identify Martin outright, instead referring to him as “Client 1.” It lists a number of findings including IBHS not reporting Martin’s disappearance to Adult Protective Services, a lack of internal investigation into the disappearance, a lack of sufficient staff to supervise clients on April 5 and a lack of timely reviews of Martin’s treatment plan.
The investigation said Martin, or Client 1, left the facility sometime between 1:30 and 2:10 p.m., and no staff members saw him walk away. Directly after noticing Martin was gone, the investigation found staff searched the facility before reporting the disappearance to the director of the facility at 2:43 p.m. The director called police a few minutes later.
