Mercedes Primo was last seen at her Jersey Street home on April 2, but the teen had been in contact with her sister until last week.
Primo, the source said, told her sister that she wasn't coming home because of a family court-issued PINS (Person In Need of Supervision) warrant against her.
The sister reported Primo missing after she stopped hearing from her.
Primo is described as 5'5" and 140 pounds.
