Cleveland police have put out an alert for Benton Zastawnik.

He is 59-years-old.

His wife tells police he went missing on December 19 and never returned.

His coat, wallet, keys and phone were left at home.

Zastawnik takes medication for seizures and left his medication at home as well.

He was last seen on Plymouth Road in the Old Brooklyn area.

Still missing.

His wife, Polly Zastawnik, has been handing out fliers for the past 11 days with his picture on it.

“I’ve been working non-stop since this happened, trying to find him. So, if you see something, say something,” Zastawnik said.

What worries her most is that Ben left his wallet, money, glasses and medication behind. She’s worried about his mental health or if he was harmed.

“My biggest concern is for his state of mind because he does take medication that he is not taking now for seizures and anti-depressants, and just his state of mind when he went missing. He’s been really hard on himself,” Zastawnik said.

Ben Zastawnik, who is 6 feet tall, walked away from his Plymouth Road home wearing black biker boots, jeans, a light blue/gray sweatshirt and a black Pink Floyd hoodie.

The Big Creek Search Dog Team will continue the search in Cleveland's Old Brooklyn neighborhood on Jan. 6.

Cleveland Police are offering a $1,000 reward for the first tip that leads to locating Ben Zastawnik.

I am trying to delicately word this because I don't want to cause trouble.
Is there a way we can "apply" for an exemption regarding this as there are some unusual circumstances regarding a family member of him. Thank you.

His wife reported him missing and said he left their Plymouth Road home without his coat, wallet, keys and phone. He also left without his seizure medication.
