IMO it's probably a combination of his TBI (lowered inhibition, aggression, impulse control judgement) with his Schizophrenia, resulting in power fantasies/delusions of his being a mob boss, like something out of the Sopranos. Throw in drugs, drug dealing, having rich and powerful parents.. yeah, makes for a nice delusion
