The American College of Cardiolgy has issued a very good set of guidelines about Hydroxychloroquine useage and contraindications, including a risk stratification.

This MD's informed consent for each patient better include that risk stratification, EKG review by a cardiologist, and evaluation or screening for porphyria or G6PD deficiency.
