Dukes and Duchesses are "His/her grace", as in "Her Grace, the Duchess of [insert name of duchy]". Members of the nobility use the highest title they are entitled to use. It takes a long time to learn what title goes with whom, as well as what form of address; the queen probably took months being tutored in this discipline.
