I have now found further Ancestry records which confirm she was born as Deborah Ann Helmontaller and that she married Roger Dale Binkley in Sumner, Tennessee on 27 October 1973. Presumably she divorced at some point as she then married David Eugene Murr on 4 December 1982. So she disappeared only 2 months after the second marriage. None of this explains the aka of Hazlewood.
