Most airlines rarely put passengers on a no fly list, no matter how horrible the behavior, unless the FBI gets involved and it delves into the federal crime territory (i.e. threat against the aircraft itself or sexual assault against a crewmember) There is one exception, a Pacific Northwest based airline that just doesn't put up with that $)#@ and bars most of them from future flight. Yay!!! I've seen time and time again, where passengers violate regulations on drinking smoking and behavior are then turned over to the airport police, who then deem it a "customer serivice issue" and release them into the terminal.
It's gotten SO much worse, back when I was flying I only had to get on the PA system one time and in my best "mom" voice say "don't make me stop this airplane and come back there", much to the glee of the flight attendants (and the offending young passeners overly excited about a soccar win who promptly calmed down).
