The decedent appeared to be a possible transient and was found in a bathtub of a condemned residence.

The photos on these websites are over exposed, I found some better photographs:

Missing Since: April 07, 1994 (six months earlier) from Salt Lake City, Salt Lake County, Utah (match - 30 miles away)
Age: 24 (match)
Height: 6'0" (match)
Weight: 150 lbs. (18lbs lighter)
Hair Color: Brown/Red (match pubic hair) long (match)
Eye Color: Blue (match) - they look very brown in the photo above though!
Distinguishing Characteristics: He has a scar on his right arm. (no mention on UID)

UID has facial hair, no mention of facial hair on Jack.

My guess: Jack may have tried to lay low due to his recent drug related assault, lived as a transient for six months (grew facial hair).

Submitted. Fingerprints are available but being held elsewhere and Jack was arrested just before he went missing http://mugshots.com/US-Counties/Utah/Salt-Lake-County-UT/Jack-Jason-Simmons.7143186.html so they must have fingerprints on file. It should be a quick rule out.

That looks like a match to me. Perhaps he dyed his hair brown and grew facial hair so he wouldn't be recognized by the people who were after him and who possibly killed him.

According to this article, his parents reported him missing in 1995 and he was added to NamUs in 2012 --
