You vastly overestimate the availability of mental health professionals in general, let alone in a correctional institution. This is a nationwide problem, but Indiana is a healthcare desert. The prosecutor doesn't get to pick the medical providers that RA sees.

Respectfully, based on testimony I have seen reported the volunteer was not thoroughly analyzing the file. She noticed a name discrepancy and reported it. It would have been negligent not to do so.

My understanding is that a volunteer found the misfiled tip years later, not that a volunteer was responsible for the misfiling/mislabeling back in Feb 2017.
ETA: If it wasn't for the volunteer recognizing the mislabeled name and bringing it to LE's attention there is no telling where the case...
Per reporting from TMS a photo album with many photos of the bridge & trail were found in RA's home during the search. His daughter also had professional photos taken there. I would suspect he had walked along the bridge tens, possibly hundreds of times.
