To me it looks like scarring, possibly from rope abrasions. It’s too high up to have anything to do with her having a trach at some point.
If she married him AFTER her accident I have concerns that he is taking advantage of her in the hopes that there may be a settlement from her being hit by a car, although I admittedly have no info about the accident, who was at fault, did the driver have insurance (and/or money), if there was or will be a financial settlement, etc. If his motive for marriage was purely control/financial, I fear for her because IMO it is more likely that he has no concern for her which puts her at a higher risk of domestic violence.
Maybe the sister is the unofficial family spokesperson and perhaps better at communicating publicly and/or on social media. Since it is the sister that petitioned for guardianship, maybe the parents are older, in poor health, overwhelmed/in shock, etc.
And the order of her meeting and marrying this “man”. (Quotes are because IMO he is not really a man in the true sense of the word.)

I hope the “husband” (ex? Not worth the name husband imo) isn’t the reason her sister has to be the one petitioning for guardianship, speaking out,… as it has happened in a lot of cases that predators try to distance their “prey” from their loved ones, often the ones warning them of their love interest (for instance making sure they go to the police & asking for a restraining order. All IMOO, as i can’t imagine the parents approving the marriagr due to a: the age difference, b: the recordings. Hopefully the sister isn’t the only familymember able to step up legally

Local news in that area doesn’t report on things like car accidents. From reading the GAL petition, I got the impression that the suicidal tendencies were the result of the TBI. Though that’s just my opinion no expertise in that area.
