When you apply for benefits, the state agency already has all of your income, tax records, unemployment claims, SS payments, etc. info available on their computer database. You are then asked for documentation (even though they already know it) before you are approved. IME.
I am quite aware of entitlement programs- some more than others.

For some benefits administered by one program/municipality (county, state, fad) they don't always know as much as you might think. If your submitted documents don't make sense, it increases the odds they seek their own.

The affordable care act did streamline documenting eligibility for Medicaid for most people, because it compares records for most people with tax information up front. (They asked for Chad's after the govt report had him ineligible.) TBH, they asked the wrong question when they asked for tax infor they already had. The needed to ask for proof that the situation had changed. For example, if Chad (possibly Mark) had decreased income.
