Hi, I haven't read everything about this case but I have a comment about the quilt.
I believe that this quilt was folded or scrunched up upon itself and stored that way for a very long time. When it was opened up, I think that the fabric stuck to each other as you can see the music note fabric is attached to the second row of hexagons and covers up most of the fabric that makes up those pieces. Additionally, there is transfer of the inside hexagon pieces onto areas along the border where we are sure it should be music note material. Each layer could be carefully separated from the other to be able to identify what fabric lays below to recreate the piece digitally.
Something that may help, I am almost certain that this is an I-Spy quilt that is an alphabetical quilt. The basic 25 hexagon layout would be typical of using a fabric that represents the first letter of the alphabet for each hexagon. For example, the first hex could be fabric that depicts Apples for A, the second could be Balloons for B, Cats for C, etc. The pumpkins and star hexagons line up perfectly for this type of quilt. The other hexagons could be misaligned due to the "stamping" of the image because of the transferring of the degraded material.
BTW-For the extra letter, either the last hex would be split into two sections or a full hexagon would be put in the middle of the bottom border to complete the alphabet.
This is just my opinion, as a quilter who is the daughter of a quilter who was taught how to quilt by my great-grandma starting at about the age of 8.
