Immunizations Required For School Policy Statement And Guidelines
State law requires documentation of student immunizations. This information must be provided to Wichita Public Schools on an official form such as
 Kansas Certificate of Immunization (KCI) - available from your neighborhood school, most health care providers, and
www.usd259.org. Another states Certificate of Immunization
 School transcript containing immunization information from a previous school
 Medical records from the health department or from a medical agency
Documentation Requirements On Official Forms
**Documentation of your child's immunizations must include month, date, and year.
**Acceptable signatures include health care provider and health department officials.
**Signatures of a nurse, physician assistant, or nurse practitioner working in a physicians office will
be accepted if they include the name of the health care provider or clinic with whom they are affiliated. USD #259 School Personnel, Parents, And/Or Guardians
Are Not Authorized To Complete And Sign KCIs
The following documentation of legal alternatives to vaccination requirements are acceptable.
1) A medical exemption shall be validated annually by a physician (Medical Doctor/M.D. or
doctor of Osteopathy/D.C.) by completion of KCI Form B. This form is available at
http://www.kdheks.gov/immunize/schoolInfo.htm or from the school nurse. The medical exemption must be due to a physical condition of the child so that the tests or inoculations would seriously endanger the life of the child. KCI Form B must be renewed every school year.
2) Written statement signed by parent or guardian that the child is an adherent of a
religious denomination whose religious teachings are opposed to such tests or inoculations. The school reserves the right to require verification by the clergy of the religious tenets that preclude immunizations.
During Suspected Vaccine-Preventable Disease Outbreaks, Any Child Claiming Any Of The Above Exemptions Will Be Subject To Exclusion From School
Please Note
At least one dose of each required vaccine must be received immediately preceding enrollment. Parents must verify that the child is in progress of obtaining basic required levels and awaiting the next available dose.
New students to Wichita Public Schools awaiting the transfer of immunization records have fourteen days to obtain them.
Philosophical beliefs for immunization exemption are not acceptable in the state of Kansas.
