Nicholas Samuel Boyett was last seen at an assisted living facility located at 244 Harrison Avenue.

Boyett is believed to be suffering from dementia or some other cognitive impairment.

No photographs of Boyett have been provide to the media at this time, but he is described as a white man, about 5 feet, 8 inches tall, and 175 pounds.
Boyett has short brown hair and green eyes.

He was last seen wearing a green T-shirt, tan-colored pants and gray shoes.

Boyett may be traveling in a 2005 Ford Taurus with North Carolina tags "DCC-1278."

The N.C. Center for Missing Persons has issued a Silver Alert for a man missing from Yadkinville.
