I think with the international attention on this case Peru is being ultra cautious in how they are treating Joran and treating evidence. I think mostly the reason for the physical and the video documentation of it is for evidence purposes... looking for evidence that she fought him off or left any of her own hairs, blood, DNA on him particularly in any areas that appear she may have struck him. Wounds look different after a few days of healing... they would be able to tell if any cuts or bruises he had occurred very recently (in the time he was on the run) or likely occurred when he was with Stephany.
However, it is also possible that part of the documentation of the exam was that there was no evidence he was mistreated by police... like bruises around the wrists from handcuffs being to tight, any signs that he was dehydrated or hadn't eaten for awhile that may indicate he was mistreated by not being given meals, etc., and also to check for any illnesses, or other medical problems that would indicate he required special treatment or medical service.
I think Peru is being extra special careful in dotting all the i's and crossing all the t's so no loophole is created accidently by them that can be used in his favor at trial.
Heck, I also think Joran is currently getting the red carpet treatment. Three square meals a day, a real bed with blanket plus even the staged photo shoot for the media as customary for celebrity red carpet.
