Sure looks like an exit wound to me, upper chest, center-(his) right.
--In the first photo taken, Sweat sitting up, 2 officers with plastic gloves holding him up.....the arms of both those officers are covered in blood. What some thought looks like a protective vest I think is actually the difference in color of his camo shirt- one part is drenched in blood, the other part not. in 3rd photo, the officer is maybe taking Sweat's shirt off, at least to his wrists, which are likely cuffed (feet sure are shackled). Looks again like single shirt, same one.
--PS. Vermont State Police issued a statement praising NY LE for their work and not mentioning their own. That's Vermont.

The officer who took Matt down was 100%, for a fact, Vermont LE working Border Patrol (my SIL is Vermont LE....horse's mouth, so to speak). Given all those joining forces to help out, it could have ended up being any one from a lot of places, but....it wasn't. Good job, VT LE.
--So far all I see Cuomo calling for is an investigation led by an Inspector General, who will be tasked to evaluate...prison design (???!!) and security (getting warmer, but not by much).
The story is already being shaped, and an ending proposed.....that Tillie and Palmer were the only two involved, that it was the first escape in a zillion years (read between the lines- nothing really wrong at Clinton, these guys just got lucky, and even then it took the help of TWO bad apples, not just one, for them to escape).
---If Sweat survives AND returns to Clinton, hope media's eye brows are raised , and if survives, returns to Clinton AND is put in general population , you know someone pretty high up wants him dead. Hope the right folks are still paying attention a few months from now, if Sweat lives.
