"DHEC also announced another child in South Carolina has been diagnosed with Multisystem Inflammatory Syndrome in Children (MIS-C), which is associated with COVID-19. The child is between the ages of 10 and 17 and lives in the Upstate, where three other recent MIS-C cases have also been identified."

Multisystem Inflammatory Syndrome in Children (MIS-C)
This report is updated on the first and third Friday of each month.

Health Department-Reported Cases of Multisystem Inflammatory Syndrome in Children (MIS-C) in the United States

As of 8/20/2020, CDC has received reports of 694 confirmed cases of MIS-C and 11 deaths in 42 states, New York City, and Washington, DC. Additional cases are under investigation.
  • Most cases are in children between the ages of 1 and 14 years, with an average age of 8 years.
  • Cases have occurred in children from <1 year old to 20 years old.
  • More than 70% of reported cases have occurred in children who are Hispanic/Latino (239 cases) or Non-Hispanic Black (197 cases).
  • 99% of cases (687) tested positive for SARS CoV-2, the virus that causes COVID-19. The remaining 1% were around someone with COVID-19.
  • Most children developed MIS-C 2-4 weeks after infection with SARS-CoV-2.
  • Slightly more than half (55%) of reported cases were male.

Next steps
Children appeared to be less likely than adults to be infected or to have severe illness early in the COVID-19 pandemic; however, as the outbreak has progressed, larger numbers of children are getting infected. It’s unknown whether this increase in COVID-19 cases among children will also increase cases of MIS-C. CDC and state partners will be monitoring for additional cases and will adapt MIS-C recommendations as needed.

CDC investigators are assessing reported cases and children’s health outcomes to try to learn more about specific risk factors for MIS-C, how the illness progresses in children, and how to better identify MIS-C and distinguish it from similar illnesses.

Case Definition for Multisystem Inflammatory Syndrome in Children (MIS-C)
  • An individual aged <21 years presenting with feveri, laboratory evidence of inflammationii, and evidence of clinically severe illness requiring hospitalization, with multisystem (>2) organ involvement (cardiac, renal, respiratory, hematologic, gastrointestinal, dermatologic or neurological); AND
  • No alternative plausible diagnoses; AND
  • Positive for current or recent SARS-CoV-2 infection by RT-PCR, serology, or antigen test; or COVID-19 exposure within the 4 weeks prior to the onset of symptoms
iFever >38.0°C for ≥24 hours, or report of subjective fever lasting ≥24 hours
iiIncluding, but not limited to, one or more of the following: an elevated C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), fibrinogen, procalcitonin, d-dimer, ferritin, lactic acid dehydrogenase (LDH), or interleukin 6 (IL-6), elevated neutrophils, reduced lymphocytes and low albumin

“They're calling it multisystem inflammatory syndrome in adults, or MIS-A, and say it's similar to multisystem inflammatory syndrome in children or MIS-C. Like MIS-C, MIS-A is not obviously linked to coronavirus and sufferers may not show any other symptoms that would point to Covid-19 infection.

New studies from India show that children do get CoVid at the same rates as any one else - and, based on their size, they also spread it (they're not as tall as the best spreaders - and they don't have the lung capacity of athletes, but they spread roughly as much as adults).

The studies are on the ground, empirical and very large compared to most others. India also has some natural social distancing built in (high caste people doing way better than low caste people - and almost no low caste people were living to be past 60 anyway).

A rare Covid-19 complication was reported in children. Now, it's showing up in adults.

"This needs to be in the forefront of every intensive care unit physician's mind," said one expert.

It was a rash that tipped Dr. Alisa Femia off.

Femia, director of inpatient dermatology at NYU Langone Health in New York City, was looking at a patient's chart, which included several photos of the 45-year-old man who had, in recent weeks, cared for his wife while she was sick with Covid-19. The man had dusky-red circular patches on the palms of his hands and the soles of his feet. His eyes were pink, and his lips were extremely chapped.

His body was erupting with the kind of extreme inflammation noted almost exclusively in children at the time.

"Before I even saw the patient," Femia recalled, "I said: 'This hasn't been reported yet. This must be MIS-A.'"...
