This is such a horrible thing, my prayers for the doctors' families and also for their patients, as it takes awhile to build up a trusted relationship with a therapist and they will be suffering also.
I feel the suitcase was left there to throw off LE; we're all familiar with the astronaut who had the adult diapers etc in her car. Wheeling in the suitcase itself may not have aroused suspicion. I have seen reps for medicine companies come in with them stock full of freebies pertaining to new medications on the market. The man who did this, took pains to do things to throw suspicion away from himself. Dr. Shinbach did not know him. The killer told him "i know where you live" after ripping his wallet out of his pants. The wallet had a driver's license and he also took money. The killer was yelling about not being helped or something to that effect. It seems to me like he wants to appear to be a patient to those who would have heard it.
He is a dangerous sociopath. I think police are on the right track with this PA guy. A sociopath can go through 10 hours of questioning and act like they don't have a guilty thought on their conscience. There have been cases of serial killers being interviewed by police, bluffing, being released and later caught; for example Arthur Shawcross.
The killer had an obsession with Dr. Faughey for some reason. His obsession made it personal, with the overkill and violence committed during the murder of Dr. Faughey and the attack of Dr. Shinbach.
One minute we have a man sitting in the waiting area chatting nonchalantly with a patient who was waiting, before going into Faughey's office. A half hour later there were screams.
"Faughey's receptionist had gone home and a female patient was waiting for Shinbach. The killer - wearing a green coat and sneakers - chatted with her for a time and then suddenly went into Faughey's office, sources said. About half an hour later, Shinbach heard screams and ran to his colleague's aid."

I don't think he was a regular patient of Faughey's as he seemed very unconcerned about chatting it up with a patient who was waiting for Shinbach. He didn't seem to fear being recognized. He also must have been acting calmly and not arousing any suspicion until he could get into Faughey's office.
LE has the right idea in looking at the man from PA as he did say he had been emailing and phoning Dr. Faughey more often. This could have been a build up of his obsessions. Not to mention the diapers being sold only in that area of PA.
I think the only reason he didn't kill the woman who kicked him in the groin, is since he knew he was pressed for time to get out of there. The killer is a sociopath and is laughing probably thinking he got away with the perfect crime, when in reality he will be caught. Most likely due to his own cockiness.
